Well, that depends. Would you call eating a large number of chocolate chip cookies (made and given by a coworker as a birthday treat) for supper (nothing else, just the cookies) as 'doing something special'? ;D
Birthday celebrations in my office are very big deal. You come in, in the morning, to a colorful "Happy Birthday" banner hanging over your desk, and a sign on the door outside the office, so everyone else knows to wish you Happy Birthday.
Mid-morning there is great feasting and singing and a card signed by everyone in my department. Once the feast is over, if there's anything left, the birthday celebrant is allowed to invite anyone else in the building they care to have partake in the leftover feasting. This year there wasn't anything I wanted to share, I took the cookies, fruit and bagels home. The cookies because they were made specifically FOR me. The fruit salad because it is free food that will cut my grocery bill for a few days, as I have been craving fresh fruit for weeks and don't have the energy to make fruit salad. The bagels because I never eat them, so they are a nice treat to have for a few days.
