Title: Mazy cat's dental
Post by: Middle Child on July 19, 2015, 02:53:00 PM
Mazy cat has finally reached the point in her life when a dental is advised.  My vet has been watching her teeth very closely, and last check up in January she showed me a back tooth with heavy tartar.  She didn't feel the rest of Mazy's dental health warranted a dental yet.

But now she thinks there may be an resorptive lesion behind that area and it's time for the dental.

Mazy has not had a dental before (she is 11) but she did have GA 2 years ago (just around this time) for an ear clean out.  She does very well with the sevoflurane anesthesia.

My biggest concern for Mazy has always been her stainless steel fear, but they kept her in her carrier instead of a vet cage last time and she was not traumatized.  I don't see any reason why she can't be treated that way again, so I am, I think a little calmer this time.

Here's the hurt my heart part.  Now that I know Mazy may have a resorptive lesion (oh heck,  I know she does.  My vet knows what she sees in a cat's mouth, and I dealt with them in Tolly  angelRIP.)

Anyway I AM seeing the tell tale symptom and realize I have actually been seeing it for a few weeks.  Actually I just cheeked the journals.  Back in the middle of April there is a notation "Mazy teeth chatter? watch for it".

Yep, I knew FORL when I saw it in April, and then in May she had the ear flare up (which is gone now) but tsk tsk, if I'd remembered the chatter, remembered to watch for it, AND took her in when she had the ear flare up like my vet wanted, that resproptive lesion might have been caught two months earlier.

On the other hand, is the thought that two months ago, maybe it wouldn't have been jnoticlbe to be caught.  Nah, I can't buy that, considering we've been watching that area.  I simply goofed.

And of course knowing about the FORL also brings to my eyes other sublte changes in bheavior that indicate she's not top shape.

I would tell anyone else not to be hard on yourself.  Cats are the masters of hiding any discomfort at all let alone pain, so it's not your fault you didn't realize right away.  Even with the symptoms right in front of me.  Tsk.

Well, well, anyway vet says Mazy cat certainly isn't suffering, just a little momentary discomfort now and then,  so I am planning to schedule the dental on September 1st, the blood work is scheduled to be done August 25th.

You shouldn't be hard on yourself at all. I've now got a million coulda, shoulda, woulda's, but there just are no guarantees. When you do your best, that's all you can do, and I think Mazy's in the best hands.

If you're willing to leave your carrier there, I don't see why they wouldn't do as you ask to keep her calmer. I'd spray the heck out of it with the pheromones the night before though.

I agree with DeeDee.  And, you've also had a lot going on with your family for the last few months.  You're human.  The important thing is that it's been identified and will be treated.

I've now got a million coulda, shoulda, woulda's, but there just are no guarantees.

Been there, done that (and still do occasionally) with Pookie's cancer.  Blaming ourselves doesn't change the past.  We do the best we can, and you've done wonders with Mazy.

Mazy is absolutely in the best hands, and spraying the Feliway is a great idea.  If you think it would help, leave a t-shirt or something of yours in the carrier with her so she can smell you and take comfort from that.

Heh. Add another coal to the heap.  I also now realize I've noticed a few times that Mazy cat's breath is not as sweet as it normally is.  Not all the time just once in a while, to give me pause...then I forgot about it 'til the next time.

Don't worry I'm not wallowing in self recriminations.  But it is interesting how the mind can..side step... things that it seems like I would normally jump on.

Mazy cat had her blood work today preparatory for her dental, which is now scheduled for Monday August 17.

She did very good for getting the blood, according to the tech, I love this tech and she is amazing with Mazy, understands her so well.  Mazy cat had some rescue remedy a half hour before we left for the vet, and I wiped the carrier with feliway wipes and sprayed her towel this morning.

At the time of her dental, while she is under the vet will also clean out any wax, if any, in her ears, and express her anal glands, and even get a urine sample, she hasn't had a urinalysis in a couple of years so might as well get that done too, all while she is under the Sevo.

I was sick to my stomach at work all morning dreading this and now thank goodness this part is over, just the dental to get through, and 11 days for that to hang over my head ugh ugh!

Mazy cat is fine now that she's home has had her lunch and I am going back to work.

xx

The ducks are lining up.  Mazy cat's blood work was all fine.  The usual aberrations, BUN and Creatinine just slightly elevated, also cholesterol, but vet and I have agreed that this is due to the raw diet.

Darn I meant to ask her if they are using the new IDEXX testing.
 http://parenting-furkids.com/index.php?topic=3805.new#newIDEXX

I haven't seen a copy of the bloodwork yet, and her last (in January) doesn't indicate what lab does the testing.

I have spoken to her at great length and all is planned out.  I do not have to do the usual "drop off between 7:30 and 8:30".  My vet has agreed that I can bring Mazy cat in at 11, which will be nearer the time she will be taken in for her procedure.  This means no hours crouched in a carrier terrified.

I have taken a vacation day from work, so as to reduce my own stress.  Feeding the other two while fasting Mazy is the only issue.  I may make them all wait.  It will be difficult, but still easier on Mazy than sitting in her carrier at the vet for three or four hours.

While Mazy cat is under she will have her dental and extraction (if needed).  She will also get her ears cleaned out of any wax, have her anal glands expressed, and have a urine sample taken (and sent out for culture).  All under anesthesia, so no stress for her!

Vet wants to use the ketamine induction before using the sevoflurane.  I balked, but she looked up Mazy's ear procedure from two years ago and told me she had used it then also.  I said I hadn't thought to ask then because I thought she didn't use ketamine with sevo.  She said she never did with Ootay because of her age, or Tolly because of his seizure disorder, but she thought it would be less stressful for Mazy cat to have the sedation cocktail first before being intubated.

Since Mazy has already had it with no ill effects, I reluctantly agreed. I reminded her though no metacam, and no convenia.

For pain meds and antibiotics (in case of extraction) the estimate has Simbadol (opiate that works the same as buprenex, can't get buprenex any more, I didn't think to ask why on the phone) and clavamox drops.  I said I thought I would prefer baytril and vet hesitated, she doesn't like to use baytril in cats with the exception of really necessary cases. I sighed and agreed to clavamox, but pills not liquid.

So that's it, all ready to go.  Oh man I wish it was over already.

So glad you've got all the plans laid out now. It's too hard to go through things when plans aren't complete.

. . .the estimate has Simbadol (opiate that works the same as buprenex, can't get buprenex any more, I didn't think to ask why on the phone)

Is that not what Vlad had for pain in emergency? He had a shot of  "buprenorphine."

Was it not possible for Mazy to be the first morning appointment?  Either way, that is AWESOME that she won't have the stress of waiting.
Is having her ears cleaned and anal glands expressed the norm?
The things you think of to ask about... and asking for pills instead of liquid...you just amaze me.  As always.

Thanks you guys.  Lola, no there were already a number of surgeries on the schedule for Monday morning, Mazy couldn't come first.  Boohoo!

I have requested the other things: anal glad expression, ear clean out and urine culture, because those are all things that cause Mazy such extreme stress.  I just think it makes sense to get them all done while Mazy cat is under anesthesia. They are not routine under anesthesia procedures.

I am anticipating that miraculous change after dental that so many people witness (including myself). I've noticed for a while now that she is often sitting UNDER things, especially after eating. I am sure this is because of mouth pain making her feel vulnerable.  And she hardly plays at all.  She acts interested, but then stops almost immediately.  I think her mouth hurts and has hurt for several months.

I am grateful I was able to take the day off from work. This is why I hoard my vacation days haha!

I was wondering if ear cleaning and anal glad expressing were routine (or should be)... in general?

If you're talking raw fed dogs, Lola, it's rarely done. It's just not an issue when an animal has the right poop going through to stimulate the anal glands to express the lubricating oil.

Ear cleaning, well I have a weekly routine for that too. It's easier to keep them clean with a good cleaner to alleviate the need for a complete ear flush by a vet than it is to risk an ear infection by letting their ears get dirty down deep. I'm more inclined to believe that routine ear cleaning is more important than worrying about anal glands because raw food directly affects the anal gland health.

Both cleaners I use are listed for dogs AND cats:

http://www.drugs.com/vet/earmed-boracetic-flush.html

http://www.jefferspet.com/products/earwash-teatree-oil

I found that using one of them every 3 days with Dannyboy ended the ear infections, and the vets have never had any remarks about dirty ears since I've continued it for years. Just rotate which you use every 3 days.

They called and asked me to bring her in a little earlier.  She's been there since 10:30.  I can't settle, on anything.

This morning, every time I woke up, I rolled over and went back to sleep, trying to spend as much time in bed as possible, to delay the "Not feeding of the cats".  The last time I forced myself back to sleep was about 8:00 and I dreamed about Tolly  angelRIP.

The dream was extremely convoluted, as dreams are, but the Tolly part was very clear, I was taking him in for a dental, but (without going into dream details) I was sitting on the outside step of a house (not my house) and Tolly was under the bushes across the yard and I called to him to come because it was time to go.  He ran over and jumped right up on my lap and I was stroking him and telling him about the dental.  Right in the middle of that my conscious mind intruded and I woke up thinking "wait a minute it's not Tolly, it's Mazy going in today."

It was 8:30 and I stumbled blindly out of bed still thinking about Tolly. After  minute I woke up enough to think about the dream, and I tried to see if there was any kind of ominous message, but aside from the confusion the first part of the dream had brought on (nothing to do with the cats) there was no feeling of anything bad happening.  So I concluded that Tolly just came by to remind me that he'd been through this type of ordeal many times and always was fine, and Mazy cat would be too.

I hope.

I also hope she is under right now getting it all done.

I can expect a call anytime between 12:30 and 2:30, so I was told.  (they wait until she wakes up from the anesthesia to call, I think)

Thank goodness I took this day off!

Thank you so much for waiting with me you guys.  It helps a tremendous amount to know you are out there, thinking about us.

She's all done!  Two extractions, two others were just sharp points with gum growing over them,  vet smoothed them down. Not enough in the bladder for a urine sample, ears were okay (did not need cleaning) anal glands expressed.

She can come home at 3:00.

Clavamox pills, simbodol pain meds.

We're home!  Of course she's staggering all over the place. Including up and down the stairs. Partly I'm sure because she still has the affects of the anesthesia, plus the pain med, but also she seems to feel the need to mark everything with her scent, rubbing her face and body on EVERYTHING!  funny2

I've given her a very tiny amount to eat, canned food well watered with SEB added, she's had about 1/4 teaspoon, and I wil continue to offer her those tiny amounts for the next few hours, and then a regular meal later, once I am sure she isn't going to puke.

She finally stopped staggering for a moment and I caught this shot after she flopped down.  But within a minute she was back up and stumbling down the stairs.  I followed her down but she just wanted to rub her face and body on everything and I was about to pee my pants so I ran back up.  By the time I got out of the bathroom she was back upstairs and on the Princess's bed (that was Ootay Princess's spot, now she and Queen Eva share it but it is still called the Princess's bed)

I'd better go spoon feed her another 1/4 teaspoon.....

Thanks you guys.  Mazy cat is feeling much better.  She's not staggering as much when she moves around, and is settling down a bit.  She's had about a half ounce of food in the last hour and 15 minutes.

My only worry now is the pain meds.  It's a 24 hour drug, so once a day.  She had it at 11 a.m, right after her dental.  I told the tech I had to leave for work at 7:30 and wouldn't be home for lunch until 1:30, so was it better to give to her early, or late.  She said to give it to her early.  But of course I worry about her having it when I can't watch her.  I'm just going to have to trust, I guess. Ugh.

It's not good to give pain meds late. They have stopped working, and it's harder to get them out of pain then.  2cents

Thanks Dee, yeah I did know that, that's why I asked the tech, and of course the tech confirmed it.  Still it's very nerve-wracking to give a cat a strong opiate and then walk out the door for 6 1/2 hours. Ugh!

Honestly, if it's not time-released, in the past I've split 1-a-day doses of pain meds into 2-a-day. Seems to keep them out of pain and keeps the medicine in their bloodstream stable but bypasses a lot of that "stoned for the first half of the day" effect.
Title: Re: Mazy cat's dental
Post by: Middle Child on August 17, 2015, 05:37:59 PM
That's good advice, but unfortunately it is in pre-measured syringes, and it's only a .1 ml dose.  There really isn't any way to split it in half.

Another report, Mazy had five tiny spoon fed meals and enjoyed them.  However when I got up to feed the other two their supper, she went up to her usual Meal Spot (she eats on the kitchen table) and said she was ready to eat on her own.  I didn't give her very much, first just about .1 (one tenth) ounce and then .25 ounce.  She ate both meals, but did fall off the table in between servings, poor thing.

Queen Eva, ever ready with a puffed tail...when Mazy fell off the table: (darn it, she would have to stand right in front of my stepper, so you can't really see how puffed the tail really is! Phooey!

I also forgot to mention that she went downstairs and had a nice poop early on, don't know how she snuck that in without me noticing, LOL!  This was when she was still very staggery. I looked up to see her gone from the last place she had flopped and kind of panicked.  I rushed down the stairs to find her sitting down there looking quite pleased with herself, some how she'd gotten back down the stairs, up on the littler platform, over the side of the high sided box, and held steady long enough to take a nice hair filled poop.

Mazy is doing great! This morning it's clear the pain is all gone.  Yesterday morning, before her pain med dose she was sitting under the table.  This morning she is thundering around the house, and now is in her crows nest.  I just warned her there will be no more spoon feeding at lunch time, haha! Today is the third day of the clavamox though so if she is going to have trouble with it I will know soon.

I only just realized another sign that she was having mouth pain, she had stopped playing her favorite middle of the night Game! (picking up her toy in her mouth and carrying it around the house singing)

Thanks!

Thanks Dee.  Mazy cat has discovered her inner kitten!   I've experienced this phenomenon before with cats and dentals, and I've advised hundreds of people about it.  But it never stops being amazing and astounding, the difference in a cat, especially an older cat, after a dental.

And here's something else interesting.  Queen Eva is still very lively at five years old.  However, she had slowed down a bit over these past months, and I attributed it to getting a little older and more sedate, maybe.  She's also been very clingy with me which I attributed to her being my little Comfort Cat after my mother's death.

Perhaps those things are true but since this morning when Mazy cat started Thundering Around The House, Queen Eva also is a kitten again.  She is so happy it's just incredible.  When I was in the shower she played a Game I'd forgotten about it's been so long since she played it.  That is simply she brings her toys to the closed bathroom door and plays with them until they inevitably get poked under the door.  I used to all the time get out of the shower to find a row of toys on the floor all lined up in front of my side of the door, just out of paw's reach.  Often that paw would also be stretched poking and grasping, under the door.

Now I wish I hadn't put it off as long as I did!  WOW!

I've been playing Frisbee game with them for a half hour now.  I started with Queen Eva but Mazy cat quickly barged in and took over.  She hasn't played Frisbee Game in ages and ages!

In case you're wondering what was happening with Jennie all this time, she was sitting on the Play House roof watching and periodically letting out a very loud meow to remind me that she wanted her Game too.

After the Frisbee Game Mazy positioned herself on the bench and started eyeing Jennie.  Wish I'd had the camera handy.  Mazy stared at Jennie until Jennie noticed her staring.  When Jennie saw Mazy, her ears went back a little and I could see her brace herself and Mazy crouched just the tiniest tiniest bit. So she was ready, when Mazy pounced (it's about a 3 foot leap) Jennie flew.  It was hilarious!

Okay okay, Jennie's turn, she's hollering......

That's wonderful!!!!!  I'm so glad Mazy's feeling better, and Eva's happy her friend is better.  It sounds like you have your hands full with a bunch of crazy kitties!   thumbsup1 thumbsup1 thumbsup1 thumbsup1 thumbsup1

Finally talked to the vet.  Mazy cat had FOUR resporptive lesions.  Matching on either side of her mouth.  On the upper left and right, full extractions of the middle molar.

One the bottom left and right, in the coinciding spots there are these little "premolars" the vet called them, which were almost completely reabsorbed already, the gum growing over.  There was just a little root left on each, and she removed the root.

There were no wax balls or signs of infection in her ears.

Bladder was empty

Anal glands were pretty full.

Mazy is doing great.  No trouble with the clavamox at all.
